Creating a Form Template

Purpose Use the Builder tab to create and organize a form template. The canvas provides fields for template metadata, sections, and questions, with controls to preview, copy, delete, and save or publish the template.

  • Header actions

    • Back to Templates: Navigates to the form template list.

    • Form Link and Form Settings: Open separate workflows. See Form Linking and Form Settings for details.

    • Manage Dynamic Questions and Bulk Edit Questions: Open separate workflows. See Manage Dynamic Questions and Bulk Editing Questions for details.

  • Form metadata

    • Form Name, Form Title, Form Description: Enter template information. Changes are stored when a Save action is used.

  • Sections

    • Each section includes a Section Title field and a section toolbar for managing the section and its contents. See Section information for details.

  • Questions

    • Each question appears as a card that includes a question identifier and a question toolbar for adding, moving, duplicating, editing, or deleting questions. See Question information for details.

Right-side quick actions and inspector pane

  • AI Form Builder: Opens the AI-assisted builder. See AI Form building for details.

  • Manage Dynamic Questions: Opens the dynamic questions workflow. See Manage Dynamic Questions for details.

  • Arrow icon: Opens the inspector pane for contextual details.

Preview, copy, delete, and save

  • Preview: Opens a full-form preview in submit mode.

  • Copy: Creates a new form template duplicated from the current one.

  • Delete Form Template: Opens a confirmation dialog. Deleting removes the template from associated items and archives it; it will not be accessible in the user interface but can be restored through a request to the help desk.

  • Go to Top: Scrolls the canvas to the top.

Save options

  • Save as Draft: Saves changes without publishing to end users.

  • Save as Revision: Saves a new version of an already-published template without publishing it.

  • Save and Publish: Publishes the current version to end users.
