Introduction
Forms are used in the Redlist platform to collect data from end users. These forms can be used to perform inspections on equipment, submit requests for work that needs to be performed, reporting safety issues, and so much more. This article will walk through the basics of how to create a form, and how to give users permissions to view and complete the form
Steps to Create a Form
- Click on the "Forms" button in the navigation bar on the left side of the screen
- Select "Manage" in the pop out menu that appears
- Click on the white and blue "Create Form" button in the top center of the page
- Begin building the form by giving it a Form Name, Form Title, and Form Description (if needed)
- Form Name - What users see when they are looking at the list of forms that they have access to. Make sure it is something your users are familiar with and will understand.
- Examples - DVIR, Daily Pre Op, JSA, Etc
- Form Title - What is seen at the top of the form when it is being completed by users and what is used for the PDF output of a form. These are typically more detailed than the Form Name.
- Examples - Daily Vehicle Inspection Report, Daily Pre Operation Inspection, Jobsite Safety Analysis, etc
- Form Description - Found at the top of the form below the Form Title when it is being completed by users. This is used to tell users the purpose of the form, or to give instructions on how to properly complete the form
- Form Name - What users see when they are looking at the list of forms that they have access to. Make sure it is something your users are familiar with and will understand.
- Add necessary sections and questions to the form

- Save the form by clicking on either the blue and white "Save" button at the top or bottom of the Form Builder page, or by click on the red and white "Save" button in the bottom right corner of the screen. (Note - the red and white "Save" button will move with you as you scroll through the form and will always be found in the bottom right corner of the screen
Giving Permission to Submit a Form
Now that the form is built, you can determine who you want to be able to view and submit the form. To do this,
- Click on the grey and white "Form Settings" button at the top or bottom of the form builder
- Scroll down to the access section of the window that pops up
- Click on the "User role access" check box
- In the dropdown below the checkbox, select the user roles that you would like to have access to the form
- Click on the blue and white "Save" button in the bottom right corner of the window
- Click on any of the "Save" buttons in the form builder
This form will now be visible to, and will be able to be completed by any user that has the user role that access was given to.
